Our goal at Wooten Dentistry is to help you reach and maintain maximum oral health. Here’s some useful information for new patients.

What to expect.

No one likes to feel rushed. That's why we give ample time for you and Dr. Wooten to get to know each other, with your first appointment usually lasting a little over an hour. We will go over any concerns you have and discuss immediate and long-term needs.

New patient, new forms.

All of our patient forms are available online. If you’re a new patient, use the link below to download, print, and complete these forms. Please bring them with you to your first visit. If you have a question, feel free to give us a call. We’ll be happy to assist you.

Do you have dental insurance?

Medical and dental insurance are different. Each dental policy is different from the next—even from the same company. To make things a bit easier, we’ll happily verify your insurance before you arrive.

Give us a call or e-mail us a photo of the front and back of your insurance card along with the primary account holder’s birthdate. Or, simply bring your insurance information to your first appointment. If you don’t have a physical card, we’ll need the policy holder’s name as well as policy and group numbers.

We're currently in-network with the following insurances:

  • Aetna PPO
  • Beam PPO
  • Cigna PPO
  • Delta PPO
  • Guardian PPO
  • Metlife PDP/PPO

Patients with out-of-network insurance plans will continue to be accepted, however these patients will be required to file their own claims.

No insurance? No problem.

You're not alone! In general, we ask for payment at the time of your procedure. We accept cash, Care Credit, and all major credit cards. Payment options are also available.

We are also happy to offer uninsured patients a $119 special for routine, bi-annual exams. This includes cleaning and x-rays.

Reach out prior to your visit and we’ll make sure you know what to expect before arriving.

Sometimes you’ve got to cancel.

Life is unpredictable. There are times when you can’t keep a scheduled appointment. Don’t stress. If you need to cancel, please allow 24 hours’ notice so we can offer the time to another patient. Cancellations within 24 hours or less require full payment prior to rescheduling.
